Prior to your first session with Kaleidoscope Therapy Services, LLC (KTS, LLC) please review and sign the mandatory disclosure form below.  In addition print copies for your own personal records.  The release of information form allows me to speak to a person you have designated.  Without a release of information I am unable to share or disclose any information regarding your treatment or services.
